Chapter 159 - 0153: Don't Come Anymore

Chapter 159: Chapter 0153: Don’t Come Anymore

Every surgery began on the dot, as accurate as a clock.

The anterior cruciate ligament reconstruction surgery began promptly at half past eight.

To perfect this surgery, Yang Ping used his extra 6,000 points, and spent 2,000 of them in the system space to buy another hundred of the anterior cruciate ligament reconstruction procedures for specialized training, progressing this operation from skilled to perfection level.

Director Han was extremely confident in Yang Ping. With every operation, he managed to achieve both speed and quality, flawless.

However, curiosity always gets the better of humans. The previous meniscus suturing surgery was not enough for him, for this anterior cruciate ligament reconstruction, Director Han decided to go up and observe again.

Huang Jiahui didn’t show up. Zhou Can made several calls, but she didn’t pick up. Zhou Can placed the sterile packaged surgical materials on the instrument trolley.

Not seeing Huang Jiahui, Song Zimo furrowed his brows, but as Yang Ping didn’t say anything, Song Zimo didn’t plan to voice out his opinion immediately. He decided to talk to the young woman later. She should’ve notified them if she couldn’t make it.

“The duration of the surgery?”

Fatty, out of habit, would always ask this when dealing with a new category of operation.

“Half an hour!”

Yang Ping looked at the MRI of the left knee and the dual knee CT plain scan + 3D reconstruction on the viewing light. The left knee patellofemoral groove was seriously deformed, and reconstruction would have to be performed during the operation.

The patellofemoral groove is like an arch-shaped cave, but this cave had now collapsed and needed to be reconstructed. What to reference if the original shape was gone? They had to rely on the right side; the right and left sides mirror each other.

“This patellofemoral groove has a high difficulty in reconstruction!” Song Zimo pointed out the patellofemoral groove on the image.

“The patient went to another hospital and they refused surgery, fearing problems arising from a bad reconstruction. If the reconstruction is not in place, it will cause an impingement after the reconstruction, leading to a complete surgical failure; if the reconstruction is excessive, the patellofemoral joint will lose stability, and new troubles will arise.” Yang Ping explained to Song Zimo.

Song Zimo took out a folded piece of paper: “Look, this is the left patellofemoral groove that I drew based on the right knee patellofemoral groove CT 3D reconstruction, at 1:1 ratio.”

Song Zimo unrolled the paper, and Yang Ping took it over. Indeed, it was well-drawn.

If it were a simple rough reconstruction, many doctors could do that. But to draw specific and accurate illustrations based on the right side, this spatial imagination ability was absolutely rare, very rare.

This kind of spatial imagination ability is not something everyone possesses. It requires talent, one in a million.

With the ability to play a blind game of Go, for an entire morning, without missing a single piece or making a mistake, what kind of capacity would that command? A memory and spatial imagination ability that is beyond the reach of ordinary people.

Ordinary people think in language, but geniuses think in a large number of images. Song Zimo was a genuine genius, proven!

“Get a pen!” Yang Ping immediately guided Song Zimo.

Song Zimo asked Zhou Can for a blue-black gel pen. Yang Ping brought out the 3D model that had been constructed in his brain in advance. This time, it was much easier compared to the last time, the model stayed in his brain for a longer time, although there were still some slightly blurred parts. Yang Ping instantly filled in the blurred parts with reference to the CT image in front of him. It was not as effortful, although it was still difficult, at least he didn’t break a sweat.

Yang Ping drew a new pattern under his own: “When CT performs 3D reconstruction, there may be some image distortions due to pseudo shadows, and we need to correct them. Look, this part shouldn’t bulge out, it should be smooth; here, it should be a bit deeper; and this arc needs a little correction.”

Song Zimo could only force a smile. He could only draw mirror images based on CT 3D reconstruction images and could not use the original section images to do a reconstruction on the paper.

But Yang Ping could. Mr. Ning from the Overseas Chinese Building didn’t have a 3D reconstruction done at that time, only a CT plain scan was done. Yang Ping was able to hand-draw a 3D reconstruction on paper based just on the plain scan images. What terrifying mental power was required? It was beyond human ability.

But Yang Ping had done just that, and Song Zimo had seen it with his own eyes.

If Song Zimo knew that Yang Ping in front of him actually didn’t need a paper at all, as he could create designs in his mind, he would probably take Yang Ping to do a head CT and MRI scan to see if there were any chips or similar things inside his brain.

“Are you discussing the reconstruction of the patellofemoral groove?” Director Han put his phone aside on the shelf and came over.

Although Director Han had completely let go, he was still very aware of the conditions of the patients in his department. This was a sense of responsibility.

He leaned over to look at the papers. These two young men had managed to draw the correct image of the affected side’s patellofemoral groove based on the healthy side’s one. One of them even corrected the pseudo shadows in the reconstruction image.

Huang Jiahui was not feeling well all night. She had a high fever and was drowsy. She reached out for her mobile phone on the bedside table, squinting her eyes open: “It’s forty past eight! Forty past eight!”

The surgery started at half past eight, and she should have arrived at the operating room earlier. She was supposed to be at the operating room, standing next to the instrument trolley.

She regained her senses instantly, as if a bucket of cold water was poured over her head. Her muscles all over her body were sore and she had a severe stomachache.

Her brother didn’t come back last night due to some obligations, otherwise he would have woken her up.

Jiahui was on the verge of tears. She immediately got up, dragging her heavy body, rushed to wash up, but first, she had to make a phone call.

The operation had already begun, who should she call? Call Song Zimo, Yang Ping seemed to delegate these matters to him.

The phone on the table vibrated with a call from Huang Jiahui.

Zhou Can picks up the phone: “Song Zimo, it’s a call from Huang Jiahui?”

“You answer it!” Song Zimo was busy.

Zhou Can answered the call, and after listening, he relayed to Song Zimo: “Huang Jiahui says she’s sorry. She had a high fever from a cold last night, she still has a fever, and is late. She is on her way.”

“Tell her not to come!” Yang Ping had finished examining the knee joint, confirmed an anterior cruciate ligament rupture, and began to prepare to extract the tendon, the semitendinosus and gracilis from the same side.

“Little missy, you’re pushing yourself too hard, coming over despite having a high fever. You won’t need to be here for future surgeries,” Yang Ping was saying while working.

Zhou Can understood Yang Ping’s intention and relayed the message over the phone, “Doctor Yang said you won’t need to come anymore.”

The level of miscommunication was astounding. Was there a distinction between “you don’t need to come in the future” and “don’t come”?

What does not having to come anymore translate to? To put it bluntly, it means scram! You are no longer needed in this sector!

Without any prior notice, being absent from surgery is a severe mistake. One of her colleagues had made a professor irate by doing just that, and the professor responded by saying–you will not have to come in the future. In order to pacify the professor, the company simply fired that colleague.

Jiahui was so shocked that she had no desire to brush her teeth or wash her face anymore. Tears streamed down her face as she realized the trouble she was in. She quickly dialed her brother’s number in search of help, but there was only a busy tone.

With no other options, she braced herself, quickly freshened up and changed clothes.

Ignoring the aching pain in her entire body and lower abdomen, Jiahui picked up her bag and rushed out of the house. A gust of wind made her shiver involuntarily as she crossed her arms to hug herself tightly.

The movements were so smooth they seemed exaggerated. Making a mere centimeter-long incision in the inner side of the knee, Song Zimo didn’t even have time to see clearly. All he saw were forceps, a scalpel, and a tendon stripper operating in quick succession beneath the small incision.

Yang Ping took out two tendons skillfully, extracting to their maximum lengths. Both the semi-tendon and gracilis tendons exceeded thirty centimeters.

“Prepare the braided tendon!” Yang Ping handed over the two tendons to Song Zimo, and Song Zimo moved to the sterile instrument table to process the tendons.

Meanwhile, Little Five held the leg as Yang Ping continued working on the knee joint.

The camera was reinserted to trim the meniscus and started forming the intercondylar notch.

The quickly spinning drill resembled a carving knife. The severely deformed intercondylar notch of the patient served as a blank canvas, slowly taking on the shape of a perfectly normal intercondylar notch, meticulously carved out by the spinning drill.

Next, they used a plasma knife to cauterize the newly formed inner wall of the intercondylar notch to stop bleeding and to deactivate it. This prevented the intercondylar notch from regenerating bone spurs and causing re-narrowing in the future.

“Tibial locator!”

Before Yang Ping could finish speaking, the instrument was already in his hand. He shared a knowing glance with Su Yixuan and continued with the surgery.

The pre-adjusted tibial locator was positioned, and the electric drill, guided by a pin, penetrated the tibial plateau to drill a tunnel. Releasing the electric drill and handing it over to Su Yixuan, a second pin was lodged to drill a second tunnel.

“Is the tendon ready? Report the size!” Yang Ping was waiting.n/o/vel/b//in dot c//om

“Professor Yang! It’s an 8-strand, double-bundle reconstruction! Can we slow down?” Song Zimo complained, but his hands moved deftly to keep pace with Yang Ping. Being Yang Ping’s assistant required speed and precision.

“Length 8 centimeters, diameter of the tibial end 7 millimeters, femoral end 8 millimeters. Both bundles are the same!” Song Zimo managed to keep pace effortlessly.

“7-millimeter tibial tunnel drill bit!”

Varoom! The electric drill perfectly drilled two tibial tunnels.

“8-millimeter femoral tunnel drill bit!”

Yang Ping dropped the electric drill; Su Yixuan took it immediately and switched to an 8-millimeter femoral drill bit.

Without hesitation, she drilled two femoral tunnels. The thickness of the tunnel was pre-set to 25 millimeters.

“Measure the femoral tunnel’s length!”

The ruler was handed over, with the numbers already set up in reverse.

“Two tendons. Anterolateral bundle: 25 millimeters at the femoral end both in thick and thin tunnels. Posteromedial bundle: 25 millimeters in the thick tunnel and 20 millimeters in the thin tunnel at the femoral end. Prepare the tendons quickly!”

Song Zimo turned up his speed to the max, holding his breath. Just a few seconds after Yang Ping finished speaking, he had already adjusted the length of the suspensory steel plate.

The crucial part of this surgery was the shaping of the intercondylar notch and precise double-bundle tendon tunnels. Any mistakes or inaccuracies in positioning could spell surgical failure, and in some cases, result in mobility issues.

But Yang Ping finished all of these steps in just a few minutes. The rest of the procedure proceeded even faster. He inserted the tendon into the femoral tunnel, pulled out the cortical bone with a suspensory button, and then pulled back to flip the button. Then, he guided the tendon into the tibial tunnel and secured the tibial end with bioabsorbable interference screws. This was done for both tendons.

In this way, both ends of the two tendons were implanted into the bone tissue, allowing them to come back to life later and fulfill the role of the original ligament.

The entire surgery essentially replaced the ruptured ligament with a new one, mimicking the work of the original ligament.

“Is it finished?”

It was better not to look at all, each glance made his eyes flicker.

Director Han had just left to examine a CT scan for a bit. When he returned to his seat to watch Yang Ping’s surgery in detail, he was surprised to find out that the surgery was already over.

At this moment, Huang Jiahui rushed in to see everyone putting away the tools and preparing to remove the sterile drape.

It was over, completely over!

Jiahui felt a chill in her heart.
