The Mangekyou Sharingan-a coveted, ultimate evolution of the Uchiha clan's dojutsu that every member aspired to achieve.

"Akira, could it be..." Sasuke looked at Akira, his face showing shock and confusion.
"Itachi has had his Mangekyou for years, and he's used it a lot. He's already on the verge of going blind," Akira replied, choosing not to dwell on himself, instead focusing on explaining Itachi's situation.
"I see... that makes sense now," Sasuke replied, his expression darkening as realization dawned. "That's why during our fight, even though my abilities were clearly inferior to his, I felt like he wasn't at his best. So this was the reason."
"The main reason is probably because Itachi was holding back," Akira thought to himself, casting Sasuke a sideways glance.
Sasuke, it seemed, had not once considered that Itachi had been pulling his punches. Itachi truly was a masterful actor and deceiver-even in death, he had Sasuke completely fooled. But since Itachi had deceived Sasuke thoroughly, Akira chose not to reveal the truth.
Instead, he continued, "There is a way, actually, in Uchiha history, to overcome the Mangekyou's drawbacks."
"What is it?" Sasuke asked, his curiosity piqued.
"You need to transplant the Mangekyou of a close sibling," Akira explained.
"Sasuke, you're in luck. Not only did you awaken your own Mangekyou, but Itachi also had one."
"So, in theory, if you transplant Itachi's Mangekyou, you'll gain both sets of ocular power." "This will not only enhance your abilities, but also grant you eternal clarity of vision. This is what we call the Eternal Mangekyou Sharingan."
"Transplanting a sibling's Mangekyou Sharingan to gain double the ocular power and achieve the Eternal Mangekyou?" Sasuke repeated, looking surprised.
If this was true, why had Itachi never tried to take his eyes when he was on the verge of blindness?

"Got it," Sasuke nodded, not rejecting the idea.
The next morning, after everyone had woken up and washed up, Akira led them to a hospital in a nearby town.
By now, Akira and his team were within the borders of the Land of Fire.
When they heard that Akira's team, with the legendary Tsunade herself, was requesting the use of their hospital facilities, the staff at the hospital considered it an incredible honor. Soon, Akira took Itachi's body out from the sealing scroll and handed it over to Tsunade. In truth, transplanting the Sharingan was a very straightforward procedure.
Think back to how Kakashi's Sharingan was transplanted.
It was done casually while on a mission, with relative ease.
And during the Fourth Great Ninja War, Sharingan and even Rinnegan transplants were done without any surgery at all.
This has been humorously referred to by fans as the Uchiha's eyes being "plug-and-play." Even Obito, after death, was able to transfer his eyes to Kakashi in the spirit world-that was even more over-the-top.
So, it seemed that transplanting a Sharingan was even easier than a regular eye transplant. With Tsunade herself overseeing the procedure, the transplant was completed effortlessly. After resting for half a day, Sasuke had fully adjusted to his new eyes.
He opened his eyes, and they transformed into the Mangekyou state. The pattern in his eyes now was a fusion of his original Mangekyou design and Itachi's.
Upon opening the Eternal Mangekyou, Sasuke felt a tremendous surge of power-it was as if his abilities had doubled. To put it in terms of strength values for a Kage-level shinobi:
Previously, Sasuke's overall strength was around 10,000.
Now, after the transplant, his power had reached about 30,000.
The magnitude of the increase was something Sasuke could clearly feel within himself.
Of course, Sasuke's knowledge and experience now far surpassed his original state in the storyline.
Thus, even though his new strength was thrilling, it didn't make him overconfident.
In the original story, when Sasuke transplanted his Mangekyou Sharingan, he became incredibly arrogant.
He thought himself invincible-first deciding he'd deal with Killer Bee, only to be utterly beaten.
Then, he stubbornly crashed the Five Kage Summit, getting beaten by the Raikage and nearly dying from the Third Tsuchikage's Dust Release.
But now, surrounded by a team of Kage-level fighters, especially with someone like Akira, a super-Kage-level force, Sasuke understood the vastness of the world.
So, even with his new Eternal Mangekyou form and a power boost, he was acutely aware that there were still plenty of people far stronger than him, and he dared not become arrogant.
With the transplant complete, Akira's team continued their journey and finally returned to the Leaf Village.
However, upon arrival, Kakashi and the others looked stunned.
Why? Because the Leaf Village appeared to be in a state of reconstruction, as if it had just endured a massive battle.
"What happened here? Did someone attack the village? Was it Urashiki Otsutsuki?" Jiraiya asked, visibly shocked.
"Or could it be that another village attacked?" Hanabi murmured, confused.
"How strange. We went to attack the Akatsuki, but who could have dared to attack the Leaf in the meantime?" Even Naruto was baffled as he spoke.
"Let's go. Let's head inside," Akira said, showing little surprise as he led everyone into the
village.
Akira had already anticipated that Toneri Otsutsuki would come for Hinata, which was why he had left Sasuke in the Leaf.
The destruction they were seeing now was clearly the aftermath of Sasuke and Toneri's battle.
"Akira, you don't seem surprised at all," Sasuke remarked, turning to Akira with a puzzled
expression.
In response, Akira simply gave a mysterious smile, telling everyone that their mission was complete, and he needed to make a brief report at the Hokage Tower.
After leaving those words, Akira gently patted Hanabi on the head, signaling her to go check
on her sister.
Then, using Body Flicker, Akira vanished from sight.
With their return to the village, everyone else dispersed, intending to learn more about what

After all, this mission involved the life and death of his own son.
"The mission is complete. Itachi's Mangekyou has been transplanted to Sasuke."
"Clan Leader, I'll hand over Itachi's body when we reach the Uchiha district," Akira added,
understanding Fugaku's feelings.
Upon hearing this, Fugaku swayed slightly, feeling dizzy.
Still, he held himself together and gave a silent nod.
Akira knew that Fugaku needed some time alone, so without saying more, he turned and left
the Hokage Tower.
As for asking about Toneri's attack, Akira knew this wasn't the right time to ask Fugaku, so he
decided to go find Sasuke instead.
